Diocesan Priests' Reference Form - September 2003
Below is the Diocesan Priests' Reference Form issued as a result of the consultation with schools and priests throughout the diocese. This version is issued by the Archdiocese of Westminster for immediate use by schools which need it. The agreement of the dioceses of Southwark and Brentwood is being sought and notification will be posted here when that agreement is reached. This is expected soon.
There are three documents:
2. Form: Part A "Your Self- Assessment"
3. Form: Part B "Priests' Reference"
Parents need access to all three forms. Each document is designed to be printed on a single sheet of paper - the first two double sided. It would be helpful if they were copied onto coloured paper: we suggest GOLD for the reference notes, LIGHT BLUE for Part A and LIGHT GREEN for Part B. These colours are completely arbitrary, but if everyone uses the same colours this will be of enormous assistance to the priests who have to fill lots of them in!
Thank you for all your responses to the
consultation on these forms, the result of which is, we hope, a better set of
forms. We still welcome your comments on the revised format because we will
review this on an annual basis. A summary of the
